Living Situation
If the client is to be discharged after Phase I, then he or she will finalize all arrangements for the living situation to which he or she is returning and/or coordinate referrals to other levels of care. This should include a discussion with family/significant others to reinforce established leverage, clarify and solidify expectations and boundaries, and develop a detailed plan to implement leverage and apply specific consequences as necessary in the event that the client does not follow through with the agreed-upon recovery plan.
Recovery Plan
In preparation for discharge, it is essential to have a detailed and comprehensive recovery plan in place that has been developed with clinical staff, client, and family input. The recovery plan needs to include bio-psycho-social-spiritual components, aftercare, and NA or other twelve-step meetings, sponsorship, literature, etc. The recovery plan should also include referral to the LVRC Alumni group. Finalized referrals and arrangements for aftercare are based on the client's needs and circumstances. Facilitation of direct connection with an Intensive Outpatient Program (IOP) or other levels of care/services as indicated.
